OBJECTIVES:

GENERAL RESPONSIBILITIES:

SUPERVISION AND TREATMENT TEAM MEMBERSHIP

All Milieu Counselor staff (on-call and regular) are supervised by and are responsible to the residence supervisor, assistant residence supervisor and senior milieu counselor.

The regular M.C. is responsible for delegating tasks and responsibilities to on-call workers while they are working together, when senior M.C. is absent.

Milieu Counselors participate in formulation of goal/treatment plans and are responsible for carrying out goals/plans and decisions.

Milieu Counselors participate in supervisory meetings, general staff meetings, in-service training and conduct meetings with clients when needed.

Milieu Counselors take consumers to medical, psychiatric, social work and other appointments, where they will act both as advocate and communicator for and with the consumers. They are also responsible for driving the consumers to recreational outings and home visits when necessary.

MILIEU COUNSELORS ARE RESPONSIBLE FOR:

1. Communicating appropriate problems to supervisor.

2. Training consumers in daily living activities, developing self-help and social skills, and maintaining data sheets for the purpose of monitoring progress.

3. Reporting/logging incidents and occurrences.

4. Administering medications and recording such (if AMAP certified).

5. Maintaining all paperwork required by OMRDD, New York State or other appropriate agencies, as it relates to their role with the consumers.

6. Waking, toileting, bathing, dressing and undressing the consumers at appropriate times. To encourage and assist consumers who are partially able to do these tasks themselves.

7. Supervising and instructing consumers to attain independence in the areas of activities of daily living (e.g., bed-making, dusting, washing and drying dishes, etc.), clothing care and maintenance, and socialization skills. Milieu Counselor staff are to provide direct assistance to each consumer until such time as they are able to care themselves adequately; and, thereafter, to help consumers to sustain already acquired skills.

8. The supervision of all meals and preparation of meals when food service employees are not scheduled or absent. (If a replacement cannot be provided.)

9. Providing a reasonably tidy and orderly living environment. This may include such tasks as laundry, after meal clean-up, making consumer’s beds and cleaning or changing a consumer who soils.

10. Boarding consumers on appropriate transportation for their Program.

11. Ascertaining whether or not a consumer is too ill to attend Day Program and, if so, follow-up with appropriate medical care.

12. Administering appropriate medication to each consumer in accordance with Doctor’s prescriptions; logging dispensation of medication. (If AMAP certified)

13. Directing or physically assisting in all ADL areas and logging any significant incidents.

14. Assisting in clothing maintenance, repair and shopping.

15. Other duties as assigned by supervisor pr assistant supervisor.

ACTIVITIES AND RECREATION

Milieu Counselors are responsible for providing educational and recreational activities for after program, evening and weekend hours.

Milieu Counselors help consumers plan outings and accompany consumers on recreational, cultural and educational outings. Opportunities to visit local shops for household and personal shopping should be part of the activity program.

Milieu Counselors participate in any special programs which are designed to further develop skills and independence (e.g., behavior modification program.)

All of the above are accomplished via an interdisciplinary approach.

ADDITIONAL RESPONSIBILITIES OF NIGHT MILIEU COUNSELOR

1. Nightly ½ hour – 1 hour room check of all consumers.

A registered nurse (RN) assigned to a facility is responsible for the complete nursing care and the performing of all routine medical procedures within the scope of the nursing practice for the clients under his/her charge.  The RN generally reports to the Nursing Supervisor, however, in all matters of medication is supervised by the attending physician or administers the medication in line with an external physician’s orders.

 

RESPONSIBILITIES include but not limited to:

 

  • Perform nursing assessment on each client upon the client’s entry into the program and (at minimum) annually thereafter.  The purpose of the assessment is to determine the clients' general health condition, identify any risk factors which may exist, and provide care for any presenting or out-standing medical issues.  At the conclusion of such an assessment, the RN may refer the client to a physician for further follow-up treatment, if required.

The Senior Milieu Counselor will perform all duties performed with the title Milieu Counselor. Work with children or adolescents in a residential treatment setting and is responsible for ensuring that those portions of the treatment plan which deals with day to day and independent living skills are implemented.

RESPONSIBILITES include but not limited to:

• Assist the Team Leader in the orientation of new Milieu Counselors.

• Lead at least one weekly community meeting.

• Act as liaison between milieu staff and the Administrator on Duty regarding treatment planning and implementation.

• Represent Team Leader at meetings, when designated.

• Ensure the proper implementation of the Apartment routine and structure when the Team Leader or Milieu Supervisor is not present in the facility. [Please note: The Sr. Milieu Counselor will display leadership skills in this area; not provide direct supervision]

• Responsible for day by day care of the children in the program including supervision of personal hygiene, social guidance, use of leisure time, and food planning and preparation.

• Develop, supervise and run activity sessions

• Accompany clients on trips and outings and supervise activities

• May be called upon to work one on one with a client who is displaying significant stress or assist with management issues as they relate to clients.

• If required, perform therapeutic restraint should a child be out of control. All employees are required to attend appropriate restrain training prior to performing such a restraint

• Drive the child to medical, social or other necessary appointments or assist in returning clients to the campus who are AWOL

• Shares their experience and observation of the children under care with the treatment team and immediately brings to the attention of the supervisor any unusual issues.

Project Hope is a program that provides free crisis counseling services to persons, families, and groups most affected by Hurricane Sandy. Project Hope is funded by the Federal Emergency and Management Agency (FEMA) and is administered by the New York State Office of Mental Health (OMH) in conjunction with NYC Department of Health and Mental Hygiene.

 

The Crisis Counseling Assistance and Training Program (CCP) aims to provide immediate behavioral health support, primarily relying on face-to-face contacts with survivors in their communities over a specific period of time. Provided services will be culturally and linguistically competent and include outreach and engagement in community settings; education and training; assessment and referral to mental health and substance abuse programs.

 

The incumbent will report to the Clinical Coordinator, NYC Project Hope. This is a grant-funded position. The initial grant period will run through December 29, 2012 with an anticipated total duration of one (1) year. This Position will be located in Brooklyn (office TBD) with outreach to the Coney Island community.

 

RESPONSIBILITIES:

 

  • Provide clinical and management supervision to outreach workers
  • Oversee a team of four to eight crisis counselors
  • Supervise and assists in the assessment of persons who require traditional mental health or substance abuse treatment
  • Directly conduct outreach, counseling, education, provide information and referral services
  • Build relationships with community based organizations, faith-based groups, and local agencies
