Project Hope is a program that provides free crisis counseling services to persons, families, and groups most affected by Hurricane Sandy. Project Hope is funded by the Federal Emergency and Management Agency (FEMA) and is administered by the New York State Office of Mental Health (OMH) in conjunction with NYC Department of Health and Mental Hygiene.

 

The Crisis Counseling Assistance and Training Program (CCP) aims to provide immediate behavioral health support, primarily relying on face-to-face contacts with survivors in their communities over a specific period of time. Provided services will be culturally and linguistically competent and include outreach and engagement in community settings; education and training; assessment and referral to mental health and substance abuse programs.

 

The incumbent will report to the Team Leader, NYC Project Hope. This is a grant-funded position. The initial grant period will run through December 29, 2012 with an anticipated total duration of one (1) year. This Position will be located on CI, location TBD.

 

 

RESPONSIBILITIES:

Working in a residential program, the Custodian is responsible for ensuring client  safety and the security of the program’s grounds, cottages, buildings and offices.  Additionally, the Custodian provides direct care and/or recreational services to the clients as needed/directed.

RESPONSIBILITES include but not limited to:

Regularly inspect the grounds of the facility, including parking facilities, vehicles, non-residential buildings (i.e.: storage facilities, cafeteria, gymnasium, etc.) to ensure that no unauthorized personnel, activities or immediately hazardous conditions (such as fire, burst pipes, etc.) exist.

Regularly inspect all residential and office buildings to ensure that no unauthorized personnel, activities or immediately hazardous conditions (such as fire, burst pipes, etc.) exist, and, in regards to residential sires, ensure that all residents and workers are on-site and accounted for.

Should an unauthorized individual or activity be observed by the Custodian, the Custodian should follow the appropriate facility procedure for securing the site, and immediately make a full report to the individual designated as in charge of the facility.

Immediately inform the director, supervisor, or the employee designated as being in charge of the facility of any irregularities noted during such inspections.

Assist counselors in emergency patient care situations. Such assistance may include the restraint of a child, if needed, or to request additional help through radio, phone, or direct contact.

When required, transport clients to emergency health care facilities.

Pick up AWOL residents, should they require transportation back to the facility.

Conduct routine fire and disaster drills.

Provide coverage for residents should a scheduled worked not be able to perform their duties.

Collect all paperwork from the cottages/units for insertion into the central log. Provide whatever written reports, logs, etc. may be necessary as a result of their work and/or observations.

Reporting to Clinical Director of Westchester DD, the Habilitation Specialist has responsibility for provision of program/consumer clinical treatment. Maintains agency and OPWDD regulations with regards to providing care and treatment of consumer’s clinical needs.

RESPONSIBILITIES include, but are not limited to:
1. Develop treatment goals/outcomes for all individuals
2. Monitor individuals’ progress or lack thereof. Facilitate through short range objectives to attain long range plans.
3. Regularly meet with clinicians and medical staff to ensure relevance of treatment plans. Also to discuss issues with treatment team clinicians as they arise.
4. Meet regularly with all individuals as a group to air grievances, discuss “house issues,” establish “house rules,” run consumer groups as necessary – e.g., self protection, sexuality training, anger/stress management, and social skills.
5. Provide training to individuals in the areas of rights and grievance process.
6. Conduct initial assessment of individual’s abilities and establish treatment goals.
7. Provide annual and quarterly reviews of progress in treatment goals related to behavior and daily living.
8. Present annual report which addresses individuals’ present level in the areas of cognition, sensor motor skills, communication skills, social skills, emotional skills, and activities of daily living.

OBJECTIVES:

1. To collaborate effectively, as part of the treatment team, with clinical, medical, educational, and administrative staff toward providing developmentally disabled consumers with quality care.

2. To assist each consumer in reaching his or her maximum potential, including the goal of independent living when possible.

3. To meet the emotional, physical, recreational and intellectual needs of the consumers in our care.

GENERAL RESPONSIBILITIES:

SUPERVISION AND TREATMENT TEAM MEMBERSHIP

All Milieu Counselor staff (on-call and regular) are supervised by and are responsible to the residence supervisor, assistant residence supervisor and senior milieu counselor.

The regular M.C. is responsible for delegating tasks and responsibilities to on-call workers while they are working together, when senior M.C. is absent.

Milieu Counselors participate in formulation of goal/treatment plans and are responsible for carrying out goals/plans and decisions.

Milieu Counselors participate in supervisory meetings, general staff meetings, in-service training and conduct meetings with clients when needed.

Milieu Counselors take consumers to medical, psychiatric, social work and other appointments, where they will act both as advocate and communicator for and with the consumers. They are also responsible for driving the consumers to recreational outings and home visits when necessary.

MILIEU COUNSELORS ARE RESPONSIBLE FOR:

1. Communicating appropriate problems to supervisor.

2. Training consumers in daily living activities, developing self-help and social skills, and maintaining data sheets for the purpose of monitoring progress.

3. Reporting/logging incidents and occurrences.

4. Administering medications and recording such (if AMAP certified).

5. Maintaining all paperwork required by OMRDD, New York State or other appropriate agencies, as it relates to their role with the consumers.

6. Waking, toileting, bathing, dressing and undressing the consumers at appropriate times. To encourage and assist consumers who are partially able to do these tasks themselves.

7. Supervising and instructing consumers to attain independence in the areas of activities of daily living (e.g., bed-making, dusting, washing and drying dishes, etc.), clothing care and maintenance, and socialization skills. Milieu Counselor staff are to provide direct assistance to each consumer until such time as they are able to care themselves adequately; and, thereafter, to help consumers to sustain already acquired skills.

8. The supervision of all meals and preparation of meals when food service employees are not scheduled or absent. (If a replacement cannot be provided.)

9. Providing a reasonably tidy and orderly living environment. This may include such tasks as laundry, after meal clean-up, making consumer’s beds and cleaning or changing a consumer who soils.

10. Boarding consumers on appropriate transportation for their Program.

11. Ascertaining whether or not a consumer is too ill to attend Day Program and, if so, follow-up with appropriate medical care.

12. Administering appropriate medication to each consumer in accordance with Doctor’s prescriptions; logging dispensation of medication. (If AMAP certified)

13. Directing or physically assisting in all ADL areas and logging any significant incidents.

14. Assisting in clothing maintenance, repair and shopping.

15. Other duties as assigned by supervisor pr assistant supervisor.

ACTIVITIES AND RECREATION

Milieu Counselors are responsible for providing educational and recreational activities for after program, evening and weekend hours.

Milieu Counselors help consumers plan outings and accompany consumers on recreational, cultural and educational outings. Opportunities to visit local shops for household and personal shopping should be part of the activity program.

Milieu Counselors participate in any special programs which are designed to further develop skills and independence (e.g., behavior modification program.)

All of the above are accomplished via an interdisciplinary approach.

ADDITIONAL RESPONSIBILITIES OF NIGHT MILIEU COUNSELOR

1. Nightly ½ hour – 1 hour room check of all consumers.

2. Clothing maintenance – ironing when necessary

3. Labeling of clothes when necessary.

JBFCS is seeking to fill one (1) full-time salaried position to serve as Team Leader for Project Hope.

 

Project Hope is a program that provides free crisis counseling services to persons, families, and groups most affected by Hurricane Sandy. Project Hope is funded by the Federal Emergency and Management Agency (FEMA) and is administered by the New York State Office of Mental Health (OMH) in conjunction with NYC Department of Health and Mental Hygiene.

 

The Crisis Counseling Assistance and Training Program (CCP) aims to provide immediate behavioral health support, primarily relying on face-to-face contacts with survivors in their communities over a specific period of time. Provided services will be culturally and linguistically competent and include outreach and engagement in community settings; education and training; assessment and referral to mental health and substance abuse programs.

 

The incumbent will report to the Clinical Coordinator, NYC Project Hope. This is a grant-funded position. The initial grant period will run through December 29, 2012 with an anticipated total duration of one (1) year.   • A Master’s Degree in Social work, Public Health, Public Administration, or other relevant graduate degree from an accredited college or university
  • A Baccalaureate degree from an accredited college and one (1) year of professional experience

JBFCS is seeking to fill one (1) full-time salaried position to serve as Team Leader for Project Hope.

 

Project Hope is a program that provides free crisis counseling services to persons, families, and groups most affected by Hurricane Sandy. Project Hope is funded by the Federal Emergency and Management Agency (FEMA) and is administered by the New York State Office of Mental Health (OMH) in conjunction with NYC Department of Health and Mental Hygiene.

 

The Crisis Counseling Assistance and Training Program (CCP) aims to provide immediate behavioral health support, primarily relying on face-to-face contacts with survivors in their communities over a specific period of time. Provided services will be culturally and linguistically competent and include outreach and engagement in community settings; education and training; assessment and referral to mental health and substance abuse programs.

 

The incumbent will report to the Clinical Coordinator, NYC Project Hope. This is a grant-funded position. The initial grant period will run through December 29, 2012 with an anticipated total duration of one (1) year. This Position will be located in Staten Island (office TBD).

 

RESPONSIBILITIES:

 

  • Provide clinical and management supervision to outreach workers
  • Oversee a team of four to eight crisis counselors
  • Supervise and assists in the assessment of persons who require traditional mental health or substance abuse treatment
  • Directly conduct outreach, counseling, education, provide information and referral services
  • Build relationships with community based organizations, faith-based groups, and local agencies
  • Provide survivors with information and education about typical reactions, helpful coping strategies, and available disaster related resources
  • Other duties as assigned
